Why should I come to Crossroads?
We are a manually based, “hands on” approach to rehabilitation of physical and occupational therapy. Manually based programs assist in pain relief, acute and chronic conditions quicker, getting you back to an active lifestyle faster.

What are your hours of operation?
Monday- 7:00 am to 6:00 pm
Tuesday, Thursday- 8:00 am to 6:00 pm
Wednesday, Friday- 7:00 am to 4:30 pm

What do I need to get started?
It is recommended that you have a prescription from a physician, podiatrist or dentist to begin treatment. Managed care insurances require a referral whether paper or electronically provided by your primary care physician.Although referral from your doctor, dentist or podiatrist is preferred, state laws now allow patients direct access to care for 30 days. Your family doctor will be notified on your evaluation date and every 30 days thereafter.
Please remember to bring your prescription and insurance cards to the first visit.

How much will my therapy cost?
Most insurances cover the majority of your therapy costs. Managed care programs require co-payments to be paid on each visit. We will verify your insurance on the initial visit and inform you of your out of pocket expenses.
Payment plans can be established for qualified individuals.

How long will each visit take?
The initial evaluation generally takes one and half hours. Every treatment following the initial visit generally lasts an hour.

How do I get to your office?
Duncansville location: Conveniently located across the VA Home between Duncansville, Hollidaysburg and Altoona. Turn at the light across from the VA Home into the Allegheny Professional Centre.Martinsburg: Come to the main intersection in Martinsburg and turn towards the Blair County Airport. We are located across from the Village of Morrison Cove Nursing Home.
